How Do I Receive a Dog Tag?

As a part of your Unchained Leader journey, each member is eligible to receive a free dog tag upon joining the program.

This guide will walk you through exactly how to claim yours by completing your Member Profile, which is the last step in your welcome email. Many members miss this step—so if you haven’t completed your Member Profile yet, this guide is for you.

Before You Begin

You must be an enrolled member in the 90 Days To Freedom Program

• Decide whether or not you'd like to receive a dog tag in the mail.

Step 1: Open your welcome email from the Unchained Leader team

• Search your inbox for the subject line: “Welcome to Unchained Leader! It's Time to BREAK FREE!”

• Locate the email you received right after you joined the program

• Open the email and scroll down to Step 5 (Or last step in the email)

Step 2: Click on the link provided in Step 5 to fill out your Member Profile

• Tap or click the link labeled “Complete My Members Profile” Or Click Here

• This will take you to a short form to fill out

Step 3: Fill out all the required fields on the form

• Enter your full name

• Confirm your email address

• Provide your correct shipping address

• Double check for spelling errors to ensure successful delivery


Be sure to select the option "Yes you can send items to my address". 

Step 4: Submit the form

• Click the “Submit” button at the bottom of the form

• You’ll see a confirmation message when your profile is submitted successfully

Step 5: Wait for your free dog tag to arrive

• Once your form is submitted, our team will process your request

• Your dog tag will be mailed to the address you provided

• Shipping time may vary depending on your location (please allow up to a few weeks delivery time. Each dog tag his handmade individually, and orders may back up occasionally.)

Important Notes:

• If you do not want a dog tag or free gift in the mailed, please select the options "please do not mail me items" when filling out the Member Profile form.

• If you already received your original dog tag and are looking to purchase a second or replacement, click here to order another dog tag.
